Report A Driving Concern

A dedicated form for reporting concerns specifically about a BSS driver, vehicle, or transport shift. Treated with the same seriousness as any complaint. Anonymous option available.

If this is an active safety emergency, call 000 first.

This form is for post-incident reporting. For an accident in progress, immediate injury, or criminal behaviour (e.g. suspected drink driving), contact emergency services first β€” then submit this form.

All driving reports are reviewed by a team leader within 1 business day. Safety-related reports are actioned immediately. Anonymous reports are treated with the same seriousness as named ones.
